Lake Geneva Therapy accepts numerous insurance plans and provides both in and out-of-network billing services for our clients.
Lake Geneva Therapy requests that each client contact their insurance company to inquire about their benefit information (below is a guideline to use when contacting insurance companies). It is suggested that this is done prior to the initial visit. Please bring this information and your insurance card to your initial visit.
Once benefits are quoted our billing service provider, CSRI will file all claims and work directly with the client to manage their accounts.
Guidelines for obtaining your benefit information:
Call the “members” toll-free number usually located on the back of your insurance card and ask the following questions:
- How many sessions are allotted per calendar year
- Is there a co-pay or co-insurance and if so, how much is it per session
- Is there a deductible and if so, how much is it and how much has been met to date
- Is an authorization required to see a therapist

Your place of work determines your benefit package and therefore LHA cannot quote your co-pay amount without first contacting your insurance company with identifying information.
Be sure to contact your human resources department to inquire if you have an employee assistance benefit, which includes a number of therapy sessions free to each employee. Not all companies offer EAP benefits.
Lake Geneva Therapy does not currently accept State insurance plans (Badgercare, Forward Health or State funded UHC).
Lake Geneva Therapy offers a sliding fee for clients who do not have insurance benefits. The fee is based on your income and number of dependents.
